65 Falcon tag decode

Can anyone decode my door tag for me? My dad just gave me this Falcon. My grandma bought it new!

R = Dark Ivy Green Metallic

I'm not positive on the trim, but I think it's Palomino and Med. Palomino - Cameron Cloth and Vinyl (Bench)

date is January 21st

DSO is Charlotte
