holy SHIZNIT !!!!! thats pretty wild what has been causing that ? It sounds like a diesel ,,,,,,,,,, but the extra fuel thats burning off , caused by the engine idle brake or engine brake ?

Firing on probably half the cylinders... fuel is just burning once it gets to the air and the other exhaust sets it off.
Bad coils would be my guess, and an over rich condition caused by the lean readings - lots of O2 passed by the non-firing cylinders.

No wonder that thing won't run. Somebody put ignition coils and O2 sensors on that diesel.

Those damn new fangled diesels with ignition systems. I'll never understand 'em.

The short version is that the flames come out of the exhaust during the particulate trap cleaning process. Fuel is injected during the exhaust stroke which travels to the particulate trap and burns the soot out of it so that the trap does not fill full of soot and cause an exhaust restriction. The trap is necessary to meet emissions. The recall is a simple reflash of the PCM.
